I seen an LP of Prime where the player died at Flaaghra. He was doing fine until he fell into the poisonous water and couldn't get out.

Posted this elsewhere but, not thinking of using powerball on the queen, I had completely forgot about it, I had low health at that point of the battle so when I retried I only had a little time before I died to try and kill her, so for 5 or 6 tries I just spammed bombs as fast as I could then it suddenly clicked...

In Prime 2 Echoes, at the Vault where you get the Screw Attack, I kept dying at the hands of the Rezbits until I realized I could do an Entangler+Missile thing. 8B It felt great once I managed to get the Screw Attack, then I could kill those automated monsters... >:)

When I first played corruption v.s Rundas. I freaked out when he would throw huge icicles at you, so I ended up getting hit by ice to death.. :/

When I was a kid, I kept getting pwn3d by Flaaghra... then Thardus... then Omega Pirate (but he kills every player at least once, I think.)

Of course, I was just an 11 year old and had the reflexes and cognitive ability thereof.

Not sure if this counts, but just started a game of metroid prime on hypermode and ran into a pirate who was limping on Frigate Orpheon. I tried to scan him and fired a few shots at me that I wasn't worried about because I thought that I'd be able to kill him easily. So I read the scan and it says more or less, "Space pirate has recieved minimal injuries and is incredibly dangerous, recommend use of heavy firepower." O_O So at this point I'm like oh crap because I realize that I'm down half of my only tank. I eventually killed him and had to be extremely careful to not die before making it to the save station. So I didn't die, but its embarassing how close I came on the very first section -_-;

As a kid, i died at Flaahgra to the point where i gave my friend a Yu-Gi-Oh card to beat it for me,
I was a kid after all, but it's still pretty embarassing.

In newer times,
I lost to :kraid: in Zero Mission, MULTIPLE TIMES.
Died at the black pirates in the final escape sequence since i didn't think i could kill them.
I also died at the Omega Metroid in Fusion since my mind went into Hyper-beam mode and
the thought of actually charging the beam didn't occur to me until i was near death.

Oddly enough, In ZM on my Hard mode playthrough, i died to :kraid: yet not :ridley: ....... maybe due to having a shitheap of energy tanks and lotsa missiles, but still...

Yeah, the Other M elevator, although with that one my reaction was "What the eff!? Instant death traps do not belong in Metroid! They better not throw in quicktime events next" and then I died at the first quicktime event with the giant lava worm thing. God, I hate quicktime events. God of War was a great game, but it spread a really annoying gimmick across the video game multiverse.

But the most embarrasing death was dying from fuel gel on Bryyo in Prime 3. I had a lot of energy tanks so I figured I could last long enough to wade through the stuff for a missile tank. Imagine my surprise to find that the health loss accellerates ridiculously fast after a few seconds.
"I'd better wait until Samus kills that Metroid before I shoot her in the back. After all, I don't know if my freeze gun will work on it."

Exactly!! Quick time events Don't belong in metroid!! When I first played other m, with the lava monster, I fell in and samus got eaten up by the monster.. My sis was laughing at me -_-;
